Supported Independent Living

Supported Independent Living is long term supported accommodation in a positive home environment, encouraging you to be independent.

 

Supported Independent Living will give you the opportunity to:

• Socialise and build relationships

 

• Be involved in recreational activities of your choice

 

• Learn new skills

 

• Build your independence

Short Term Accommodation & Assistance

Short Term Accommodation & Assistance is what used to be respite for people with disabilities.

 

Short Term Accommodation & Assistance will allow you to experience some time away from your family home, in a safe, supported shared accommodation home.

 

You can use Short Term Accommodation in one of our shared accommodation homes or in your own home.

 

At our Short Term Accommodation & Assistance homes you will have the opportunity to:

 

• Enjoy new experiences

 

• Socialise and meet new people

 

• Be involved in recreational activities of your choice

 

• Learn new skills

 

• Build your independence

 

• Participate in the community

 

You can stay for the day, overnight, a weekend, a holiday, or longer according to your funding and our availability.

Support Coordination

Support Coordination will assist you to develop an individualized plan and start organizing your supports and connect you with your local community and services.

Support Coordination will help you to:

 

• Plan your goals

 

• Find services that can help you with your goals

 

• Choose the services you would like to use

 

Specialist Support Coordination

A Specialist Support Coordinator is an experienced practitioner who will:

 

• Assist you in building your NDIS individualised plan

 

• Work with you to access health, education and justice services

 

• Help you understand what special supports you can access with NDIS

 

• Ensure you have the specialised services that you need

 

• Assist you to book appointments for your special supports

 

• Work with you to achieve your outcomes.

Specialist Disability Accommodation

Specialist Disability Accommodation is a form of housing that is designed to enable you to receive the support you need to live more comfortably in your own home.

The type of build will depend on your disability.

Specialist Disability Accommodation funds will pay for the building that you live in.

Social & Community Participation

Community Access is when a support worker helps you go out in the community to do things that you like, such as:

 

• Go out for shopping

 

• Visit your favourite places (coffee shop, library, cinema)

 

• Do activities in the community (classes, sports, concerts)

 

• Visit friends and family

 

• Go to work or volunteer

 

• Visit health professionals

 

With active support from AGAPI Care support workers, you can:

• Achieve your goals and outcomes

 

• Engage in recreational activities like going to the movies, joining in sporting activities, attending classes and workshops, walking to local cafés and shops

 

• Do your own shopping and budgeting

 

• Manage your personal care while out and about

 

• Learn to travel on trains, trams and buses

 

• Interact with your community with confidence

 

• Reach your potential

Plan Management

AGAPI Care’s Plan Management service can help you look after your NDIS money.

 

We will help you:

 

• pay invoices

 

• do the paperwork and

 

• keep records about your NDIS money and how much you have spent.

 

You can choose the service providers you want.

 

Your service providers can be registered with the NDIS or not.

 

As your Plan Manager we will:

 

• look after your NDIS money

 

• pay your support invoices

 

• look after your NDIS paperwork

 

• keep records about what happens with your NDIS money

Independent Skills Development

AGAPI Care will help you with personal daily life tasks so that you can live as independently as possible.

 

These supports can be provided in a range of environments, including your own home.

 

AGAPI Care will assist you with therapy supports and functional assessments and implement strategies to meet your goals according to your NDIS plan.

 

We can also support your with training in mobility and strengthening and improving daily routines to enable your maximum independence in your daily life.
